Health Risk Assessment (HRA):
Step 1: Visit http://www.thedialogcenter.com/bcbsnd/
Step 2: Login or register using your nine-digit health insurance ID card number (do not enter ‘YQA’)
Step 3: Click on the “Health Tools” tab at the top of the page
Step 4: Take the “How’s Your Health” survey

Cholesterol/Blood Pressure Screening:
Step 1: View the enclosed calendar of events to find a screening time and location that works for you. You do not need to set up a time in advance.
Step 2: Attend the screening, which includes total cholesterol, blood pressure, waist measurement, height, and weight. Educational materials and referral information will be available. It is recommended that you don’t eat at least 8 hours prior to the screening.
Step 3: Read the “Notice of Privacy Practices”.
Step 4: Sign the “Acknowledgement of Notice of Privacy Practices”.
*If you have received a screening within the last 6 months, all you have to
do is provide us with proof and results to be eligible for the raffle drawings
LEARN AND PRACTICE HEALTHY BEHAVIORS (see enclosed calendar of events):
Online and paper tracking with monthly drawings. Keep track of your daily physical activity, fruits, and vegetables and you could win some great prizes. If you track online, there’s nothing to turn in. Your name will automatically be entered! If you keep track on paper logs, which are provided, you will need to turn them in by the first Friday of the next month.
