ACTIVITY LOG

Q: How do I get the tracking logs on paper?
A: You should have received the calendar logs in your registration packet. If you have misplaced them you can download the logs.

Q: Should I mail my completed activity log at the end of the program?
A: That is up to you.  The paper log is simply a tool for you to keep track of your progress.  However, at the end of each month, there will be a prize drawing for those that have used the log.  For every week that you track your progress, your name will be entered into the drawing.

Also, if you turn in your log, your progress will be entered online by Work Well staff.  A final report will be available to you at the end of the program, if you like.

Q: How do I turn in my logs?
A: You can send your log to Amanda Eickhoff, Stop 8365 through campus mail.  Logs must be received by the first Friday of the next month to be entered into the drawing.

Q: Do I have to log onto the computer daily/weekly to input my exercise information?
A: You have the option to use the interactive activity log on the computer daily/weekly to log the days you are physically active or consume your fruits/vegetables.  You can also print the activity logs on paper and keep track of your progress for the entire time.

Q: What if I am traveling but still get my exercise in and have no access to a computer to log results?
A: The interactive on-line activity log is simply an option that can be used daily or weekly input. However, we still offer the paper activity log that can be used for the duration of the program.
